With this Licensed Practical Nursing position, you will be part of a comprehensive team providing care for the individuals who call this 32-suite beautiful community home. The responsibilities of this position will include playing a meaningful role in developing a culture dedicated to optimal healthcare practices and centered on each and every Resident’s well-being.
LPN Position Specifications:
This position will ensure that the health and safety of all residents are met. The LPN will assist the RN with planning, coordinating, and managing the home care service and act within the Comprehensive Home Care regulations. The LPN is responsible for the overall direction, coordination, and monitoring of home care service delivery under the direction of the Director of Health Services.
Must have innate passion for serving people living with dementia.
Must possess the skills to communicate effectively with those you supervise, residents, family members and RN Supervisor.
Must offer experience with leadership and team oriented supervisory skills.
Must be able to prioritize and organize work effectively and efficiently.
Able to work irregular hours, occasional weekends and evenings.
Work within Comprehensive Home Care and complying with state regulations.
Must be registered and licensed with the State of Minnesota as a Licensed Practical Nurse and must maintain current unencumbered license with the Board of Nursing. Certification in the philosophy of Integrative Nursing is a significant added value – however is not a requirement.
Ability to guide the development of a continual improvement, process-driven, best practices foundation, while building a team of exceptional care providers from the ground-up.
Commitment to a compassionate care philosophy emphasizing a whole person approach.
Ability to develop and implement a quality healthcare management platform with a commitment to a quality improvement process for Memory Care; and to gather, track and analyze data related to client outcomes and other key measures.
Excellent judgment and decision-making abilities and problem-solving skills.
Ability to develop, monitor and work within a budget.
Ability to incorporate up-to-date, evidence-based research into the Memory Cares’ procedures and protocols.
Extensive experience with EHR/EMR (electronic health/medical records), ideally PointClickCare.
Ability to manage all aspects of a fully integrated Memory Care program including Resident intake, Resident care, personnel management, customer relations, complaint resolution and policy development.
Ability to schedule nursing employees to ensure adequate staff coverage per established staffing pattern and budgetary guidelines.
Support the orientation and education requirements of staff.
Ability to work with a team to develop person-directed care and outstanding sensory and cognitive programming, in a culture of compassion, respect and dignity.
Must receive a “not disqualified” criminal background result from Department of Human Services and may not be on the OIG exclusions list.
Must be screened for TB.
